The WORTH Transitions team completed a multi-site implementation trial testing the efficacy of this risk reduction, trauma-informed, intervention for drug-involved, formerly incarcerated women being released and linked to a specialty health care clinic for follow-up.
WORTH Transitions is a self-paced application, and also group facilitated by a community health worker who has lived experience. It is for women of all race/ethnicities and sexual orientations with a special focus on women of color.
